We are excited to work with you. In preparation for completing the Onboarding process, below is a list of items that will need to be reviewed. All documents below must be submitted in PDF Format Only.
Registering with Colorado Community Management (“CCM”) requires that the Vendor/Contractor pay a non-refundable Registration and Onboarding fee of $150.00 prior to being listed as an approved vendor for CCM. These fees will be processed through CCM’s online vendor registration portal.
Vendor/Contractor will also pay an annual renewal fee of $150.00, due each year on the anniversary of the initial registration date, which will be automatically billed by the site. Fees paid for Onboarding, Registration, or Renewal are non-refundable.

W-9 Form – Complete a W-9 and have it ready for upload in the portal.
Certificate of Insurance – Insurance Requirements:
Carriers must carry an AM Best rating of A- or better.
Commercial General Liability (CGL) - $1M per occurrence / $2M general aggregate
Automotive Liability - $1M combined single limit per accident covering owned, hired, and non-owned vehicles
Workers’ Compensation - Coverage as required by the State of Colorado with employer’s liability limits not less than $500K per accident.
Banking Information – CCM prefers to pay Vendors/Contractors via ACH to prevent lost checks. Please have a routing number and account number ready for registration.
Applicable Licenses – If you are a licensed trade that requires licensure, please have a copy of your license available.
Other Documents - If you have other documents, you would like CCM to review as part of your submission, please have them ready as well.
